Phase logs

The working record of every development session, in order: what was asked, what was built and why, the alternatives that were rejected, the bugs found, and how each change was checked. They are the long form behind the short phase summaries and the decision records.

Lines about the maintainer's own machines, paths and accounts are left out; nothing else is edited except links between logs.
